Beschrijving

Lazertinib (YH25448,GNS-1480) is een potente, zeer mutant-selectieve en irreversibele EGFR-TKI met IC50-waarden van respectievelijk 1,7 nM, 2 nM, 5 nM, 20,6 nM en 76 nM voor Del19/T790M, L858R/T790M, Del19, L85R en Wild type EGFR, en toont veel hogere IC50-waarden tegen ErbB2 en ErbB4.

In vitro

Lazertinib richt zich op de activerende EGFR-mutaties Del19 en L858R, evenals op de T790M-mutatie, terwijl het wild type wordt gespaard. In NSCLC-cellijnen en primaire kankercellen van patiënten met EGFR-mutaties remt YH25448 de groei van kankercellen potenter en verhoogt het de apoptose van tumorcellen significant in vergelijking met osimertinib. In de celproliferatietests zijn de GI50-waarden van lazertinib respectievelijk 6 nM, 5 nM en 711 nM voor H1975-cellen (L858R/T790M), PC9-cellen (del19) en H2073-cellen (wt).

In vivo

In een in vivo muismodel geïmplanteerd met H1975-cellen, resulteert eenmaal daagse YH25448-behandeling in een dramatische dosisafhankelijke tumorregressie in zowel subcutane als intracraniale laesies zonder abnormale tekenen zoals huidkeratose. De plasmahalfwaardetijd van YH25448 is 5,9-6,8 uur, terwijl de tumor-naar-plasma AUC_0-last-verhouding 3,0-5,1 is bij tumor dragende muizen. YH25448 toont een uitstekende penetratie van de bloed-hersenbarrière, waarbij CSF-concentraties worden bereikt die de IC50-waarde voor pEGFR-inhibitie overschrijden. YH25448 toont superieure werkzaamheid voor tumorregressie in een EGFR-mutant hersenmetastase-model.
